چکیده

I read with interest the paper by Yoo et al., published in the March, 2016 issue of the Journal, about the 41-year-old previously healthy woman, who suffered Takotsubo syndrome (TTS) complicated by cardiogenic shock, necessitating resuscitation and invasive supportive means of extra-corporeal membrane oxygenation, with eventual full recovery of her heart function. The patient’s first electrocardiogram (ECG) (Fig. 1A) recorded on her 2nd visit to the hospital, showed low QRS complexes, in keeping with a recent report, associating transient attenuation of the ECG QRS complexes (attQRS) and TTS. It is difficult to compare this ECG with the one recorded 1month after discharge (Fig. 1B), since the upper panel of the figure has half pISSN 1975-4612 / eISSN 2005-9655 Copyright © 2016 Korean Society of Echocardiography www.kse-jcu.org

Prolonged circulatory support with an Impella assist device in the management of cardiogenic shock associated with takotsubo syndrome, severe sepsis and acute respiratory distress syndrome

Severe sepsis has been known to trigger for takotsubo syndrome which is associated with profound physical or emotional stress. Severe sepsis is also associated with sepsis-induced cardiomyopathy, a reversible myocardial depression. We report a case in which a patient with takotsubo syndrome, cardiogenic shock, severe sepsis, and adult respiratory distress syndrome was managed with an Impella Ca...

Cardiogenic Shock due to Psychosis-Induced Inverted Takotsubo Cardiomyopathy Bridged-to-Recovery with a Percutaneous Left Ventricular Assist Device

Inverted Takotsubo cardiomyopathy, a less common variant in the spectrum of stress-induced cardiomyopathy, is increasingly being reported. This report describes an acute psychiatric illness leading to the onset of this syndrome. The patient presented here developed cardiogenic shock but successfully recovered with the use of a percutaneous left ventricular assist device.

Response to Letter Regarding Article, "Stress-Induced Cardiomyopathy Presenting as Shock"

First of all, we would like to appreaciate your deep interest in our case report. Fortunately, we have a previous electrocardiogram (ECG) of the patient before the symptom onset (Fig. 1). And we did check the serial ECGs during the admission. At initial admission, there were no prominent ST and voltage changes in precordial leads (Fig. 2). Follow-up ECG taken after 3 hours from the first ECG sh...

Dynamic Left Ventricular Outflow Tract Obstruction with Cardiogenic Shock in Apical Ballooning Syndrome.

UNLABELLED Apical ballooning syndrome, also called Takotsubo cardiomyopathy, is characterized by transient systolic dysfunction of mid to apical segments and hyperkinesis of basal segments of the left ventricle that mimic acute myocardial infarction without significant coronary artery stenosis. We reported a 51-year-old man with chest distress, hypotension and abnormal electrocardiogram. Echoca...

Venlafaxine intoxication with development of takotsubo cardiomyopathy: successful use of extracorporeal life support, intravenous lipid emulsion and CytoSorb®.

We describe a young patient who ingested 18 g (240 times the daily therapeutic dose) of venlafaxine in a suicide attempt. She developed severe cardiomyopathy in a takotsubo distribution causing cardiogenic shock and multi-organ dysfunction syndrome (MODS). She was successfully treated with intravenous lipid emulsion (ILE), extracorporeal life support (ECLS) and CytoSorb®. This is remarkable as,...
